Hawaii has one of the most distinctive healthcare systems in the United States, shaped by its island geography, multicultural population, and unique history of near-universal health coverage. The state has required employers to provide health insurance to employees working more than 20 hours per week since 1974 under the Hawaii Prepaid Health Care Act, giving it one of the lowest uninsured rates in the nation long before the ACA.

The majority of healthcare services are concentrated on Oahu, where The Queen's Medical Center in Honolulu is the state's largest private hospital and a Level II Trauma Center. Straub Medical Center, Pali Momi Medical Center, and Kapiolani Medical Center for Women & Children (all part of Hawaii Pacific Health) serve the Oahu population. On the neighbor islands, Maui Health System (operated by Kaiser Permanente) and Hilo Medical Center on the Big Island provide regional care.

Hawaii's Med-QUEST Division administers Medicaid and CHIP, covering eligible low-income residents. The state uses a federally facilitated marketplace for ACA plans. The Hawaii Department of Health (DOH) (health.hawaii.gov) oversees public health programs, and the Hawaii Medical Board licenses physicians.
